Dear Parent/Guardian:
On behalf of the staff at Bradford District High School, it is my pleasure to welcome your child for the 2020-2021 school year. As
with any start up, there are variety of required and option forms to fill out. This year, we have some forms that are paper hardcopies, and
others that are to be completed online.
Paper Forms:
Included with this letter will be the Student Information Verification which you are required to either complete or revise. We ask
that your child return the signed form to their period one teacher by Monday, September 14th, 2020. The forms that need to be returned
• Student Information Verification
• Each year a new Emergency Medical Plan form is required for students with medical concerns. If your child has a
medical condition, we need to know. You can obtain this form on the Bradford High School website or obtain one from
the main office. It is important that you sign and date the bottom of the form.
Online Forms:
School Cash Online is a computer application authorized by the school board to track permission forms and allow for online
payments (which are highly preferred to other means of payment). If you are not already registered, we request that you do so right away.
Instructions are at the bottom of the letter. These forms are required to be completed using the School Cash Online system by Monday,
September 14th, 2020.
• Legal Custodian Volunteer Transportation Consent
• Student Information Computing Technology Appropriate Use Agreement
• Student Permissions and Acknowledgements
• Student Accident Insurance
The School Cash Online application will also be needed for giving permission and paying for any optional school activities
such as field trips or sports (when we are able to have them).
For families that do not have access to the internet, we have a front desk computer in the office that can be used for accessing the
internet and School Cash Online. If you have no internet and can’t get to the school, please phone the office and request assistance.
Regarding attendance, research indicates that the level of student achievement is directly related to attendance. Whenever your child
will be absent from school, please telephone the school office prior to 8:30 a.m. on the day of the absence. Attendance calls can be made any
time of day or night by utilizing our voice message system by calling the office at (905)775-2262 and pressing “1” for the attendance line.
With the pandemic situation, plans for School Council Meetings and our annual “Welcome BBQ/Parent-Teacher-Student Night”
are still tentative. More information will be posted on our school website at as it becomes available.
